2) Photocopy of Birth Certificate
3) Photocopy of Status Card or Letter from Home Nation.
4) Photocopy of Health Card
5) Registration Fee.(Cash Only). $40.00 CDN, or $30.00 USD
6) one Dark Jersey and One light color Jersey
7) Full Pads
Please show up 1 hour early to allow time for complete registration process.
